Responsible for providing physical therapy for outpatients with timely documentation. Performs initial evaluation, develops treatment plan and provides therapy as needed. Other duties as assigned.

Requirements
MINIMUM QUALIFICATIONS/COMPETENCIES:
Must
be a licensed Physical Therapist in New York State and one year of post-licensure experience.
PREFERRED QUALIFICATIONS/COMPETENCIES:
1-year clinical experience; strong organizational
skills; good communication skills; experience in outpatient programs;
familiarity with outpatient regulations; and documentation requirements. Experience working with a team and strong
ability to monitor therapy progress and create home carryover programs.
