What are the responsibilities and job description for the Case Manager position at Holistic Wellness Solutions?
We are looking for a clinical case manager to provide mental health support services to our patients. It can be part time or full time employment, or contract position and you could work as many hours per week as you would like (it would be then heavier focused on productivity reimbursement model). We would prefer someone with a bachelor degree in human sciences, ideally in social work or psychology. Experience in mental health is definitely a plus. You get paid per hour and a part of your salary comes from productivity driven model. Ideal candidate will need to know how use EHR (Therapy Notes), will be a quick learner, and get along with the team as well as with patients. We currently provide a lot of tele health services and so ideal candidate would be comfortable with using technology to connect with patients, but will also need to come in person to provide in person services if needed (may need to drive to patient's homes or meet with them in the community to assist with various tasks). We have one opening for teen/adolescence case manager and one opening for adults case manager, however, your caseload may be mixed, so it would be good to be comfortable with both age groups.
