In 1929, it became the Borinstein Home, which provided care and services to the elderly. Hooverwood Living was established at its present location in 1970 and has grown to become a well-organized and respected facility offering skilled nursing, memory care, assisted living, adult day, rehabilitation services, and hospice. We are a nonprofit facility with Medicare and Medicaid certifications.
Role Overview for the Hospice Clinical Liaison:
The Clinical Liaison for Aaron - Ruben Nelson Hospice serves to facilitate the admission of a hospice or home health potential client between their referring physician, the Aaron - Ruben Nelson Hospice outreach coordinators, family and Aaron - Ruben Nelson Hospice. The Clinical Liaison serves to educate potential patients, families and referral sources on eligibility requirements and benefits of hospice. The Clinical Liaison may provide in-service education sessions and do public speaking on hospice, home health and related topics. The clinical liaison is responsible to meet with referral sources, families and patients upon request to educate them on hospice and home health and begin some of the admission paperwork. The goal of this position is to ensure a smooth and successful transition of patients into Aaron - Ruben Nelson Hospice home health and hospice.

Requirements
Key Responsibilities/Duties for the Hospice Clinical Liaison:
1. Meets with potential patients at home, in the hospital or elsewhere to educate them on the eligibility requirements and benefits of home health or hospice.
2. Meets with potential patients’ families to help them through their loved one’s decision making process.
3. Obtains orders and collects pertinent medical information from the physician, nursing home, and/or assisted living facility.
4. Prepares the Aaron - Ruben Nelson Hospice clinical team for the new patient and sets up the formal admission.
5. Attends hospice IDG meetings and home health clinical meetings
6. Responds within 24 hours to all hospice inquiries.
7. Speaks to community groups, referral sources and other audiences on topics such as hospice, home health, advance directives, and other health related issues.
8. Works closely with the outreach coordinators to respond potential referrals.
9. Teams with the other clinical liaisons to manage the Bridge program.
10. Provides nursing visits as time allows.
11. And all other duties as assigned.
Hopsice Clinical Liaison Qualifications:
1. RN (Preferred), LPN, Social Worker.
2. Minimum of 2 years home health or hospice experience.
3. Previous Clinical Liaison experience is a plus.
4. Excellent Communication Skills.
5. Valid Driver’s License.
Physical Requirements/Working Conditions :
1. Traveling and Driving is Required.
2. Will visit and work in clients' home.
3.On call, including evenings and weekends.
