Compensation Range: $68,000 - $71,000 annually commensurate with experience

Overview:
The Mental Health Counselor provides comprehensive mental health services to clients who are both HIV-positive and have a mental health condition, many of whom also experience co-occurring substance use issues, homelessness, complex health issues, and/or identify as LGBTQ .

Responsibilities

• Conduct case-finding activities by working from reports generated from clinic records and reaching out to patients dually diagnosed with HIV and Mental Illness, who may have missed an appointment, been out of care, or have high viral loads.
• Conduct intake assessments, reassessments, and service plans.
• Provide client–centered, culturally and linguistically competent services including individual and group counseling.
• Perform crisis intervention as needed.
• Coordinate program activities with Mental Health Advocate (MHA).
• Support the delivery of preventive and primary care to enrolled clients by facilitating adherence to scheduled appointments and medication.
• Coordinate and participate in case conferences as a member of an integrated team.
• Coordinate with internal and external partners including other departments, health centers, and supportive services providers.
• Participate in staff meetings and training.
• Travel as needed to Housing Works Community Healthcare sites in Manhattan and Brooklyn and some external sites as needed.
• Monitor navigation services for each assigned client to ensure that they are conducted according to program standards.
• Ensure that all services are maintained at program capacity.
• Maintain confidential records on paper and electronically.
• Maintain confidentiality of client treatment records.
• Participate in program and agency quality improvement (QI) projects.
• Collaborate as part of a comprehensive treatment team.
• Promote awareness and understanding at the site level of available services within the Retention and Adherence Department (RAD).
• Participate in advocacy efforts to end the twin crises of homelessness and AIDS.
• Perform special projects as assigned by superior.·
