You will manage patient caseloads coordinating plan of care and discharge, and maintain full responsibility ensuring customer and physician satisfaction is achieved. Provides leadership and effective communication to ensuring that appropriate outcomes for every patient are met in the most effective and efficient manner. Negotiates among team members including physicians to resolve discrepancies in treatment plan. Conducts comprehensive team meetings and conferences regarding specific patient needs that affect cost, quality, and LOS. Serves as central point of communication. Responsible for the effective and efficient use of human, financial, and material resources. Identifies variances timely and makes appropriate strategic adjustments. Assesses the effectiveness of the patient’s plan of care, focuses on outcomes, and recommends changes to effect patient progress. Ensures appropriate level of care and length of stay as evidenced by audit. Uses intensity of service and severity of illness criteria to make decisions about admissions and continued provision of services. Assures thorough early and ongoing transition/discharge plans by collaborating with patients, families, physicians, payor, and providers across continuum of care. Position Requirements Education: Knowledge of clinical practice and case management processes normally acquired by completing a Bachelor’s Degree in Nursing with additional training/certification in case management. BSN preferred. Licensure: RN Indiana license. Certification in an area of case management is preferred. Experience: Three to five years of clinical nursing experience with demonstrated leadership and management abilities. Critical Care experience is highly desirable.
