What are the responsibilities and job description for the Youth Care Coordinator position at InnerSourced Solutions?
InnerSourced Solutions is seeking to hire a part-time, hourly
Youth Care Coordinator to work with youth (ages 6-21) and their caregivers to identify needs and goals that will strengthen the family and the youth's future. Areas of focus include education, mental health, physical health, economic needs, and positive social development. Youth and families are met with weekly on average and work towards these goals together in the community where the needs are as well as in the family home itself. Staff transport clients when needed to make sure important connections to community resources are successful.
This job has the potential to grow into full-time employment and may require some evenings and weekend hours. Priority will be given to candidates who can complete the onboarding process and training within 14 days.
RESPONSIBILITIES:
- Provide in-person case management services to children and adolescents based on authorized level of care.
- Provide linkages to services that include but are not limited to mental health services, housing, entitlements (including insurance), etc.
- Conduct intakes to evaluate client risk and assess needs as well as completing all initial documentation required by COMAR regulations for referred clients within appropriate time frames.
- Complete initial and ongoing plans of care.
- Coordinate, facilitate, and engage in family team meetings.
- Identify and provide supportive services to clients.
- Maintain timely documentation within company EMR (Insync) for each client contact, including but not limited to: contact notes, service logs, monthly reports, etc.
- Provide transportation to clients, as needed, to meet needs and goals as listed in the client care plan.
QUALIFICATIONS/ REQUIREMENTS:
- Bachelor’s degree required in social work, psychology, counseling, rehabilitation, or related human service degree.
- Experience working in a behavioral health setting providing services to clients with emotional and behavior disabilities may be substituted for education.
- Must be able to demonstrate professional interactions, language, and confidentiality practices.
- Interest in working both with children and their parents.
- Proficient with technology, as use an electronic medical records system, (Insync).
- Strong verbal and written communication and collaborative skills.

Salary is based on years of relevant/direct experience, credentials, position hired, knowledge of responsibilities and aptitude assessed during the interview.
