Program Description
Insight Housing’s VA Grants per Diem (GPD) Program is an innovative low demand/low barrier transitional housing services program for 18 Veterans. We provide comprehensive wrap around services for homeless veterans who are experiencing homelessness, substance use issues and mental health issues.
Position Summary
Under supervision of the Senior Program Manager, this position will be responsible to provide housing case management to all Veteran’s in the program. S/he will ensure that Veterans receive intensive case management services, are linked to the medical and mental health resources of the VA, and are receive assistance with obtaining income and housing.
Essential Duties and Responsibilities
- Create individual housing plan and provide on-going case management to all Veterans to obtain permanent housing placement, retention, and achieve personal and economic stability.
- In consultation with the Shelter Program Manager, develop individual client service plans, including specific goals, objectives and budget plans.
- Meet with each assigned client at least once per week to monitor client’s progress and provide ongoing support.
- Monitor arrival and departure of Veterans and ensure that they are complying with GPD regulations. In consultation with supervisor, develop corrective action plans as needed.
- Using the harm reduction model, work with clients to maintain safety and support clients on their path to recovery.
- Make referrals to community agencies for wellness and community connections.
- Maintain case management files in accordance with agency and contract compliance standards; participate in regularly scheduled internal file audits,
- Maintain program tracking statistics and assist in compiling contract and agency reports.
- Conduct crisis intervention as needed.
- Assist clients who are placed in permanent housing with housing retention activities, including conducting regular inspection of housing to monitor living conditions and assist clients to succeed in community housing setting.
- Assist supervisor with all operational tasks as assigned.
- Ensure cleanliness and general upkeep of the program.
- Attend and participate in all meetings and trainings as assigned.
- Complete and submit timesheets in a timely and accurate manner.
- Work within the framework of Insight Housing’s Code of Conduct.
- Perform other tasks as assigned by supervisor.
Qualifications, Skills and Abilities
- Bachelor’s degree in Social Services or related field, or equivalent combination of education, training, and experience
- 2 years’ experience providing case management services and working with issues of mental illness and substance abuse
- Prior experience working in a Transitional Housing or VA program strongly preferred.
- Commitment to serving homeless individuals with a focus on harm reduction model. Prior experience working with homeless and low-income individuals strongly preferred.
- General knowledge of local community resources.
- Proficiency in Microsoft Office, email, internet, and calendaring applications. Experience with HMIS data entry strongly preferred.
- Excellent verbal and written communication skills.
- Excellent interpersonal and crisis intervention skills with a can do and flexible attitude.
- Ability to work with minimal supervision, multitask, maintain confidentiality, and meet
- Ability to maintain professional conduct, attitude and appearance at all times.
Special Requirements
- This position requires frequent driving. A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies.
- Must be able to obtain background check clearance.
