Job Responsibilities
- Implement best practice nursing care in post-acute residents
- Monitor conditions and provide updates to the nursing team
- Measure and document basic vital signs of residents
- Assist with grooming, feeding, toileting, and other ADLs
- Promote a clean and safe environment through hygiene practices and clearing hazards
- Transport residents around the facility and assist with movements as needed
- Interact with residents to keep them engaged and build trusting relationships
- Follow and adhere to care plans and policies
- Bridge communications between staff, residents, and family members
