As a Palliative Care Health Unit Coordinator, you’ll help patients obtain the palliative care they need, and be part of a team to help manage their pain and chronic conditions in a home setting where our Palliative services can see them.

As a Palliative Care HUC, here’s a big-picture view of what you’ll do:
- Coordinate care and help schedule patients with our Palliative Care team
- Update medical charts and upload documentation in patient charts
- Act as the central hub for our Palliative Care program by receiving all incoming phone calls and entering referrals
- Assist our other service lines in referring and transferring incoming calls to the proper destination
- Upload patients into our EMR and obtain information including insurance information
Here are a few must-haves for our Palliative Care HUCs:
- Medical Assistant Degree
- Strong Customer Service Background
- Minimum of 1 year in Medical office coordination
- Knowledge of state and federal Palliative Care Regulations
- Ability to multi-task and work between multiple clinical platforms
- Strong use of personal time and ability to stay on task
