The Residential Care Coordinator is a para-professional position with responsibility for the general operation within a group home and/or supported apartment setting. Primary duties are to promote a supportive, family- like environment, provide supervision of Residential Care staff, Coordination of resident health care and monitoring and scheduling of systems as required to meet accreditation and state and fed regulatory requirements
Education and Experience Requirements
- Required: *Ability to communicate effectively
Driving Requirements:
- Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care.
- Must have the use of a personal vehicle.
Preferred: Current Certified Nursing Assistant certificate from the Wisconsin Nurse Aides Registry
One to three years’ experience in the human services field.

Essential Duties and Responsibilities: *
Provides Care Coordination and general direction and supervision to Residential Care Assistants and/or Care Coordinator Assistant in relationship to individual resident care and service provided.
Maintains a case load within one setting.
Communicates and maintains regular contacts and positive relations with Guardians Payees and MCO.
Notifies concerns and follow ups in relation to consumer changes, occurrences, or concerns to guardian, MCO, and Program Director.
Develops and monitors residents’ living ISP, behavioral plans, outcomes, progression and regression.
Updates Residential Care Assistants of changes in plans and holds care planning meetings as needed. Coordinates Resident overall plans of care IE: resident reviews and functional screens, medical appointments, and consent related to services.
Completes follow-up with paperwork following all medical appointments.
Completes and reviews resident documentation.
Assists in the completion of quality monitoring efforts.
Monitors Site Systems.
Maintains relationship and communication with property owners as needed within property maintenance.
Supervision of the Residential Care Assistant and Residential Care Coordinator Assistant.
Maintains regular and predictable on-site attendance
