Description
Hospice de la luz / Jet Health, Inc. is seeking a full time Licensed Vocational Nurse or Licensed Practical Nurse to join our team as an Intake Coordinator in our Albuquerque, NM hospice office.
The Intake Coordinator will be responsible for the timely coordination of the referral process including the review/response/coordination of care for patients referred to Hospice de la luz.
Requirements
- Coordinating the referral process for patients referred to Hospice de la luz.
- Monitoring, reviewing, and responding to patient referrals.
- Developing referral relationships within the community.
- Interacting with facility representatives, discharge planners, and other agencies to facilitate care initiation.
- Conducting reverse marketing in coordination with the Business Development Leader.
Qualifications:
- Education: High School graduate or equivalent, with preference for graduates of accredited vocational/practical nursing schools.
- Licensure: Current state license as a Licensed Vocational/Practical Nurse or a multi-state license under the National Nurse Licensure Compact.
- Experience: One year of experience as an LVN/LPN, with hospice experience preferred.
- Skills: Compassionate nature, ability to work independently, good communication, reliability, and organizational skills.
Essential Functions:
- Compiling patient information from referral sources.
- Entering referrals into Homecare Homebase.
- Coordinating care with nursing supervisors and the interdisciplinary team.
- Providing effective communication to patients, staff, and other healthcare professionals.
- Serving as the first line of customer service.
- Maintaining accurate records on referral sources and patient status.
- Performing reverse marketing to reinforce messaging.
- Meeting assigned key performance indicators (KPIs).
- Coordinating with other departments for insurance verification and authorization.
- Participating in department in-service trainings and QAPI program.
