Home Health Clinical Intake Coordinator
**Office position located in the Kyle, TX office**
Job Summary:
- The Clinical Intake Coordinator is a Licensed Vocational Nurse (LVN) who works as a member of the Intake team, and in conjunction with Account Executives and Operations staff to facilitate the referral intake and admissions process.
- Review incoming referral paperwork, determine clinical appropriateness for admission, and identifies missing elements required for a compliant admission process.
- Facilitates timely procurement of required referral components, including, but not limited to physician’s orders, confirmation of following provider, and clinical documentation.
- Collaborates with Scheduler to ensure adequate staffing capacity.
- Cross trains in the following areas to assist fellow team members as necessary: scheduling, clinical coordination, intake, and eligibility/authorizations.
- Participates in weekend/holiday intake on-call rotation.

– Experience:
- Home Health Field LVN: 1 year (Preferred)
- Home Health Administrative role: 1 year (Preferred)
