The Post Adoption Coordinator is available to adoptive parents in Circuit 5 (who have finalized the adoption) in need of guidance, information and advocacy after the case is closed by the adoption agency. In a proactive manner, the Post Adoption Coordinator works to resolve issues before they escalate whenever possible. The coordinator advocates on behalf of a family when needed providing linkages, referrals, support and monitoring while responding with a sense of urgency. Identifies systemic gaps and barriers and makes recommendations for solutions and/or improvement to the Assistant Director of Out of Home Care and the Senior Director of Out of Home Care.

Essential Functions

  • Create and send Newsletter to Post Adoptive Families to include educational opportunities, summer camps, tutoring, and other services available within Circuit 5 and statewide. The Newsletter is to be created and distributed semiannually and should feature at least two (2) post adoptive families within Circuit 5.
  • Send meeting notices to all adoptive families and facilitate support groups in each county with Adoption Agency Specialist to provide support, networking opportunities and education related to adoption issues.
  • Responsible for securing the location and childcare services for the Adoption Support Group.
  • Facilitate the Adoption Support Group monthly for each county and document the meeting in FSFN within 48 hours of occurrence.
  • Contact all adoptive families one year after their anniversary. The families will be offered services and address any issues and make the appropriate referrals as deemed necessary.
  • Develop a tracker to ensure all the Post Adoption Families are contacted as outlined in House Bill 7013.
  • Ensure the report for the children finalized quarterly is reviewed in advance in anticipation of the projected workload.
  • Make contact regarding services offered as part of the 1 (one) year post adoption communication program for adoptive families that reside outside the State of Florida and outside Circuit 5.
  • Coordinate with the Children’s Mental Health Specialist and/or the Kids Central Behavioral Specialist regarding the adopted child’s mental health needs and discuss all recommendations.
  • Consult and work with the Adoption Specialist, Eligibility and other departments as needed to assist families with Medicaid, subsidy, social security and other questions
  • Follows up on all inquiries with 24 business hours of receipt and ALL home visits will be discussed with the Assistant Director of Out of Home Care.
  • Review all open cases and make referrals to the appropriate services as deemed appropriate for case management services.
  • Establishes and maintains linkages with businesses, organizations, groups, vendors and individuals within the community whose support and/or assistance will add value to the agency.
  • Collaborate with Protective Investigator when investigation involves an adoptive family.
  • Make a request for services for families outside the State of Florida and contact the state agency in which the family resides to notify them that the family resides to notify them that the family needs post adoption support and services.
  • Develop a tracker in an effort to submit the following information on a quarterly basis: 1) The number of attempts made to each family; 2) The number of successful attempts to contact each family; 3) Services Requested by each family 4) Services provided to each family 5) If services were not provided to the family the reasons why they were not provided; 6) Any recommendations in the improvement in the communication of the post adoption program.
  • Provides Community Education in conjunction with Adoption Specialist.
  • Provide log of families receiving Post Adoptive Services for monthly client satisfaction survey.
  • Tracks and reports on all children with disrupted adoptions, families serviced in support groups as well as families served through general inquiries.
  • Other Duties as assigned.

Qualifications

Education & Experience

Bachelor’s degree in human services field and 5 years of experience. Adoptive parent is preferred.

Licenses & Certifications

Ability to successfully clear a Level II Background Screen. Possess a valid Florida driver’s license and be insurable by the agency’s current insurance carrier.

Knowledge, Skills & Abilities

  • Has knowledge of community resources.
  • Ability to travel throughout Citrus, Hernando, Lake, Marion and Sumter counties up to 50%
  • The ability to establishes and maintains linkages with businesses, organizations, groups, vendors and individuals within the community whose support and/or assistance will add value to the agency.
  • Communicates effectively in speech and in writing.
  • Uses correct grammar, spelling and punctuation in written correspondence.
  • Skilled in Microsoft Office (MS Excel, MS Word, MS Outlook).
  • Skilled in time management and prioritization.
  • Has a positive attitude concerning all aspect of working in a challenging environment, including significant patience and respect for children and families who can become quite angry and demanding, be sensitive to cultural needs, and have a willingness to serve as a positive member of a working team.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and to reach with hands and arms. The employee must also frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. The employee is regularly required to bend/stoop and kneel. Repetitive motion is also required. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
