Summary/Objective
The licensed practice nurse (LPN) is responsible for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse.
Essential Functions

- Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
- Perform delegated independent nursing functions using established procedures, policies, guidelines and standards as observed by the registered nurse.
- Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes
- Maintain and coordinate communication among patient/significant other, staff and physician as evidenced within medical record, client and health care team feedback.
- Assist in assuring that effective communication exists with resident/DPOA as reflected in the plan of care. Give punctual change of shift report consisting of pertinent and relevant information for residents.
- Work collaboratively with other members of the health care team in coordination of the resident's care as evidenced by other department feedback.
- Use available written and AV materials specific to resident's condition or disease. Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration or discussion.
- Assist residents, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
- Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.

Supervisory Responsibility
This position is a charge nurse position.
Work Environment
This job operates in a long term care setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with residents who may have contagious illnesses.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 20 pounds.
Position Type and Expected Hours of Work
This is a full-time position in a long term care setting that is open 24 hours a day, 365 days a year. Rotating 12 hour shifts as well as holiday and work during inclement weather will be required. Incumbent will be scheduled based on operational need (rotate shifts, standby, AA, on-call). Full-time hours in the nursing department are 3 shifts per week. Shifts may vary based on operational need. Part-time hours are under 60 hours per week. PRN hours are scheduled as needed.
Travel
No travel is expected for this position.
Required Education and Experience
- Graduate of an accredited school of Nursing.
- Valid registration as an LPN in the state.
- Completion of relevant state Board of Nursing approved.
- Current AHA Healthcare Provider training (BCLS).
