JOB SUMMARY Under the direction of the Manager of Care Coordination and Director of Quality and Performance Improvement, the Health Equity and Community Services Coordinator will serve in a cross functional role assisting the Care Coordination and Foundation teams in identifying Social Determinants of Health for our region, providing knowledge about and organization of available community resources to improve local health outcomes, collaborating with community organizations and members to provide necessities to patients through the discharge planning process, leading community projects to expand assistance, and assistance with strategic planning and partnering with Foundation for drafting grants to fund expansion of community assistance projects that will improve local health outcomes.
