Job Description: The Case Manager will be responsible for ensuring that program participants receive high-quality and comprehensive services to meet their individual needs. The Case Manager will be expected to be knowledgeable about community resources and to develop and maintain relationships with service providers to ensure that program participants receive timely and appropriate referrals.
Responsibilities:
- Conduct intake assessments with program participants to gather information about their strengths,
needs, and goals.
- Develop service plans for program participants that are individualized, strengths-based, and goal-
oriented.
- Monitor and evaluate program participants' progress towards their goals and make adjustments to
service plans as needed.
- Facilitate group sessions as needed to address common themes and goals among program participants.
- Collaborate with the Program Coordinator to ensure that program participants are receiving
appropriate and timely services.
- Establish and maintain relationships with a variety of service providers and state-level agencies to
ensure that program participants receive referrals that are appropriate and of high quality.
- Follow up with program participants to ensure that they have successfully connected with service
providers and are receiving the services that they need.
- Advocate for program participants to ensure that they receive services that are culturally responsive
and sensitive to their unique needs.
- Maintain accurate and up-to-date case records, including process notes on all interventions and
contacts regarding clients.
- Participate in regular supervision and training to enhance skills and knowledge in the field.
