What are the responsibilities and job description for the Care Coordinator position at Legacy Treatment Services Group?
The Care Coordinator ensures individuals are linked to mental health, substance abuse, medical services, and services targeting social determinants of health as required by SDOH screening. Collaborates with other programs individuals are involved with to ensure appropriate care. Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related needs of the consumers served. He or she must be able to assess and interpret data about the consumer's status in order to identify each consumer's needs and provide the appropriate care, including age-related care, needed by the consumer group.
The Diversion Support Services program works with individuals who are incarcerated in Middlesex County and released under bail reform to link them to services, including housing, financial assistance, physical health and mental health services. The program works from a peer-focused and strengths-based model to bring consumers from surviving to thriving.
Hours:
This position is Full Time, Monday Friday, with some evening and weekend hours required.

Education/Experience:
Bachelor's degree in Psychology, Social Work, or related field. Experience working with population preferred.
