POSITION SUMMARY: The Case Manager facilitates patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be directed by the Administrator, the Director of Nursing and Medical Director. As the Case Manager, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Every effort has been made to identify the essential functions of this position.   • Facilitates and actively participates in interdisciplinary planning of clinical care of patients and assists with outcome report completion.
  • Provides outcome data to payors and Physicians on as needed basis.
  • Serves as care liaison between patient, physician, care team members, and discharge planner by coordinating, monitoring, and communicating patient’s progress.
  • Assists with coordination to most appropriate care level.
  • Objective evaluation of the interdisciplinary plans, determining barriers, clarifying realistic goals, and identifying potential alternatives involving physician, team members, patient and family.
  • Completion of progress reports to physician, nurse practitioners and external case managers as needed.
  • Coordination, planning, and provision of health care services to promote necessary outcomes and timely discharge.
  • Holds resident and family meetings for discharge planning and coordination purposes.
  • Acts as a liaison between payors and decision makers facilitating a smooth transfer of information.
  • Follow up contact and communication with patient for care outcomes and needs post discharge within 48 hours.
  • Communicates discharge information to physician and payer with outcome data, as appropriate.
  • Assists patient and family with issues of reimbursement and understanding of policy values.
  • Performs other duties as may be needed or assigned.
  • Admission audit of orders & assessments
  • Discharges- print orders 24-48 hours prior. (MD sign and family education)
  • NOMNOCS (Wellmed) email and CC MD.
  • Well med IDT follow up email and
  • Psychology/ Psychiatry follow up with both.
  • Care plan meetings
  • Communicate with nurse frequently on discharges.
  • Wellmed meetings to be led by you.
  • DME/Home Health set up for Wellmed
  • Coordinate with social worker when coverage/back up needed
  • Attend Care Plan meetings
  • Coordination of triplicates
  • On call Rotation

QUALIFICATIONS:

Education and/or Experience:

  • Graduate of an accredited nursing program or background in social services.
  • 3 years of experience as a licensed nurse in a hospital, long-term care facility, or other related health care facility preferred.

Certificates and Licenses:

  • Must possess an active license to practice as a LVN/RN valid in this state.

Other Specific Requirements

  • Must provide evidence of being free of tuberculosis infection upon hire and as set forth by the policies of the facility.
  • Must exhibit strong oral and written communication skills and be able to read, write, speak and understand the English language.
  • Must possess the ability to make independent decision when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.

WORK ENVIRONMENT:

  • Works in office area(s) as well as throughout the nursing service area (i.e. nurse’s stations, resident rooms, etc.).
  • Moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Interacts with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
  • Communicates with the medical staff, nursing personnel, and other department supervisors.
  • Works beyond normal working hours and on weekends and holidays when necessary. May be on call 24 hours per day, 7 days per week.
  • Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Is subject to injury from falls, burns from equipment, odors, etc., as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.

PHYSICAL AND SENSORY REQUIREMENTS (with or without the aid of mechanical devices):

  • Must be able to move intermittently throughout the work day.
  • Must be able to cope with the mental emotional and stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively.
  • Must meet the general health requirements set forth by the policies of this facility which may include a medical and physical examination.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
  • Must be able to lift up to 25 pounds. Requires infrequent lifting of more weight up to 50 pounds. Occasional pushing and pulling of carts.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Pay: $20.00 - $30.21 per hour
