What are the responsibilities and job description for the Case Manager position at City of Round Rock?
The Case Manager is responsible for providing outreach programs and activities, providing social support and counseling to assigned clients, advocating for clients and following-up with assigned client(s) health management/care plans.
Posted pay range is the starting salary. Pay rate offered is based on experience.
- Provides case management to client population.
- Partners with client(s) to identify needs, agree on goals, choose treatment methods, and articulate specific treatment outcomes and objectives.
- Coaches client(s) in effective management of their chronic health conditions and self-care
- Assists clients in accessing health related services including obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services
- Acts as an advocate and liaison between the client/family, provider, health plan and community service agencies. Assists clients’ with scheduling appointments and assisting with completion of applications for programs for which they may be eligible.
- Documents all client encounters including contact information for both client and relevant contacts related to client care.
- Builds and maintains positive working relationships with clients, providers and community service agencies; work collaboratively and effectively as part of a team.
- High School Diploma or equivalent
- One (1) year of experience in a health and/or social services setting
- Preferred: Bachelor’s degree in social work, psychology, counseling, public health or related field
- Valid Texas Driver’s License
- Community Health Worker (CHW) Certification within 6 months of hire
- Preferred: Bilingual/Bicultural
