Scope:
The Home Health RN manages patient care in home settings, conducting assessments, coordinating with physicians, and delivering skilled nursing care.
Qualifications:
- Graduate of an accredited nursing program.
- Current RN licensure in New York.
- Prior home care experience preferred.
- Excellent communication skills.
- Valid driver's license.
Duties:
- Perform home care assessments.
- Develop individualized care plans.
- Implement the Plan of Treatment, including wound care, injections, medication management, etc.
- Provide skilled nursing care.
