Description
The Community Care Program Manager is responsible for overseeing the Care Coordinator team and Connect to Care (C2C) program which provides advocacy, education and guidance to neighbors that may be vulnerable or underserved. Of particular importance are patients experiencing substance use disorder, recent hospital or incarceration events or have other complex needs. The program not only provides direct services to these patients but also links patients to care internally, as part of integrated care at MCHC, and throughout the community in accordance with assessments. The program manager is responsible for reporting, compliance and quality assurance that maintains the integrity of the organization and the requirements of the grants involved. The overall goal of the C2C Program at MCHC is to empower patients to make informed health care decisions within the teams plan of care, and to improve health outcomes of the individuals served in a way that supports inclusion and health equity.
Primary Duties Include:
- Works closely with the Director of Specialty Services to develop program goals, celebrate program successes, and overcome challenges.
- Participates in the hiring, onboarding, mentorship, evaluation, and progressive discipline, as needed, of Care Coordinators .
- Grant Management: Tracks and reports on key program quality metrics required for grant and quality reporting; monitoring and evaluating program efficacy; successful attainment of all grant deliverables and reporting
- Provides strategies and solutions for empowering community health that addresses social determinants of health, the systemic effects of poverty, and the gross disparities that exist in our community today.
- Participates in fiscal management meetings, monitors program expenditures in accordance with budgeted allocations, confers with program personnel for budgetary revisions, and recommends budget control actions
- Develops and Participates in required trainings in order to maintain highest level of excellence as a Community Health Worker Program for the entire team including best practices in the areas of Trauma Informed Care, health coaching, MI, and health equity.
- Collaborates with MCHC leadership team to ensure collaboration, comprehensive care and to seek feedback on services provided by C2C team.
- Maintains all ethical standards of practice, practices discretion, maintain confidentiality (HIPPA and 42 CFR part 2) and demonstrates professionalism when caring for difficult patient situations. Demonstrates the ability to cope with complex situations in a professional and positive manner and maintain integrity of the department and organization.
- Supports Care Coordinators efforts on behalf of patients in obtaining resources, gaining skills and knowledge needed for self-care and informed medical decision making. Ensures understanding of all aspects of the care plan and motivates clients to actively participate in their care.
- Receives and responds to communications from patients, community agencies and providers for assigned population. Serves as an effective liaison between MCHC and other agencies in matters relevant to client care.
- Performs in-person and telephonic needs assessments, conducts individual brief action planning with patients in a primary care setting and initiates appropriate interventions. Improves access to care by making timely referrals to community resources.
- Collaborates with the Director of Specialty Services to maintain and implement standardized program materials, including orientation packets, patient education, consents, policies, and protocols.
- Facilitates the communication with the care team regarding patient status and facilitates appropriate care delivery. The care team consists of the patient, their support system, primary medical providers, nursing, behavioral health, community agencies and specialty care providers.
- Coordinates, schedules, and facilitates regular interdisciplinary case conferences/reviews, including documentation of patient care plans.
- Monitors client adherence and compliance with plan of care and clinical outcome measures; identifies when adjustments are needed or plan is complete and notifies the care team.
- Provides treatment adherence with counseling, education, and aids in medication reconciliation within the scope of practice. Works with the patient, providers, pharmacies, and insurance companies to provide continuity of medication availability.
- Oversees the program enrollment/ disenrollment process. Maintains database of clients for reporting on grant deliverables.
- Attends strategic community meetings to promote the C2C work and identify opportunities for collaboration; Maintains partnerships with other CBO’s, the CCM program, and other systems in place to enhance the care of patients.
- Follows Universal Precautions, OSHA standards and is familiar with reporting adverse conditions including injuries and occupational exposure.
- Travels to all sites as needed.
- Performs additional duties as assigned.

Requirements
MINIMUM QUALIFICATIONS:
- Excellent written and verbal skills
- Strong Leadership and Self-Management skills required
- Understanding of public health topics such as social determinants of health, health equity, trauma-informed care, chronic disease prevention, and understanding of common chronic diseases
- Demonstrated experience in one or more of following: community organization/engagement, underserved and vulnerable populations, and grants and/or program management
- Demonstrated experience in developing and supporting collaborative partnerships with multiple agencies
- At least two years’ experience managing direct reports
- Excellent computer literacy skills in MS products
PREFERED QUALIFICATIONS:
- Bachelor’s degree in nursing, dietetics, healthcare administration, public health, social work, social science, public policy, or related field; MPH, MSW, MHA, RN, RD, or CHES preferred
- Bi-lingual, English/Spanish, fluent
- Experience in healthcare especially FQHC highly desired

Salary : $75,000 - $90,000
