The Case Managers are responsible for making home visits to screen persons referred to Meals on Wheels Plus, Inc. for services, as well as making update visits to current clients. Makes special visits to clients as required. Responsible for certain notification and reporting procedures annually. Works under the direct supervision of the Assistant Director.

Pay: From $14.50 per hour
