About us
Mecosta County Commission on Aging assists seniors in remaining independent and in their own homes by providing Meals on Wheels, Transportation, In Home Services and referrals.
We are seeking a part time Outreach Worker. This position:
- Conducts comprehensive assessments of applicants concerning the physical, social and emotional functioning, medications, physical environment, economic condition, and community and family support systems of potential and accepted agency clients.
- Makes home visits and explains services. Determines eligibility for services using established criteria and makes recommendations for interventions. Assesses needs for homemaking, respite care, personal care, meals, and other services. Identifies client needs and makes referrals to other agencies or services. Maintains working relationship with other service agencies.
- Develops service plans based on input from the client and the level of client acceptance, health care providers, members of the client’s family, and members of the client’s social network. Provides assessment and service information to the client and family.
- Assists in performing outreach services for clients such as referring for skilled nursing services, recommending durable medical equipment, referring for services related to prescription coverage, and related services. Advocates for senior citizens and assists them with issues they face.
- Provides follow-up assessments and evaluates implementation of service plans in relation to the goals and objectives for the client. Reviews client condition including dietary status and medication and determines appropriate service plan.
- Coordinates client files with Coordinator through regular case reviews and performs appropriate follow up.
Education: College level coursework in human services preferably with the equivalent of an Associate’s Degree in a human services field.
Experience: One year experience working in a nursing home, extended care facility, home health agency, community health, or similar setting providing health care experience with the elderly.
Other Requirements: Possession of a valid Michigan driver's license.
