The Coordinator | Recovery Services plays a crucial role in assisting clients of the Association in obtaining the necessary services and resources to achieve and maintain housing stability.

Principal Duties and Responsibilities

  • Conduct comprehensive needs assessments and develop individualized service plans tailored to each client's unique needs and preferences.
  • Provide ongoing support, monitoring, and coordination of services for assigned clients.
  • Conduct regular client meetings, with a focus on weekly meetings until clients achieve housing stability, followed by ongoing support as needed per the service plan.
  • Maintain accurate and up-to-date case notes in the HMIS system.
  • Actively participate in bi-weekly team meetings and provide regular updates on client progress.
  • Attend weekly meetings with the Housing Intake and Referral Coordinator for clinical staffing and collaboration.
  • Connect clients with essential resources, including financial planning, public benefits, behavioral health services, family counseling, medical and dental care, education and employment assistance, transportation, and other basic needs.
  • Advocate for clients with other service providers to ensure access to necessary support.
  • Manage a caseload of approximately 15 clients at a time, as directed by the team leader.
  • Attend mandatory Association meetings and training sessions as required.
  • Conduct outreach activities as needed to identify and engage potential clients.

Knowledge, Skills & Abilities

  • High School Diploma or GED required.
  • Experience working with individuals experiencing homelessness or in mental health/recovery services preferred.
  • Excellent organizational and time management skills.
  • Strong ability to prioritize client needs and develop effective service plans.
  • Exceptional interpersonal skills and the ability to quickly build rapport with clients from diverse backgrounds.
  • Proficiency in Google Suite and Microsoft Office applications.
  • Strong computer skills and the ability to accurately enter data into the HMIS system.
  • Excellent written and verbal communication skills.

Work Conditions & Physical Demands

  • Work is primarily performed in a climate-controlled office setting with minimal exposure to environmental hazards.
  • Moderate noise levels associated with typical office equipment (printers, phones, etc.).
  • Flexible work schedule may be required due to internal and external meetings.
  • Occasional travel may be required; reliable transportation is essential.
  • Ability to lift and carry up to 25 pounds.
  • Visual and auditory acuity to identify and respond to environmental and safety concerns.
  • Physical mobility to stand or walk for extended periods (60 minutes or more).
  • Ability to safely and effectively use office equipment.
