Merrick House Neighborhood Center is a nonprofit community center with programs for youth, adult education and an early childhood 5-star rated program for children ages 0-5.
Basic Function:
Under direction, is responsible for recruiting, enrolling and retaining pregnant women, and women with children up to two (2) years old. The Community Health Worker provides care coordination and education through home visits to program participants and their families.
Responsibilities:
- Recruit and enroll women in need of prenatal care, and support services via community canvassing and other outreach efforts.
- Assist in the identification of families at risk for infant morbidity, particularly where the child shows signs of failure-to-thrive, abuse and neglect.
- Conduct home visits at client’s residents for women enrolled in the program to provide health education and facilitate risk reduction activities.
- Develop realistic and attainable goal plans with family to address child development and social and emotional growth as well as parent/child bonding and positive interactions.
- Assist parents in developing formal and informal community supports.
- Make referrals to appropriate medical or social service agencies and resources according to participant needs and risk factors. Follow up on all referrals.
- Conduct assessments on clients and infants at appropriate intervals.
- Plan, implement, and facilitate parenting education activities using an evidenced based curriculum.
- Participate in MomsFirst community activities and events while encouraging participants to attend as well.
- Maintain a set caseload which may fluctuate based on funding.
- Act as an advocate for clients.
- Travel throughout service area required.
- Complete and maintain paperwork, reports and records as required meeting all deadlines, in order to comply with audits of computer and paper files.
- Exemplify positive role modeling by professionalism, language, work ethic and positive communication.
- Perform other duties as assigned
Requirements:
- Combination of education and experience represented by an Associate’s Degree in Early childhood Development, social Work or equivalent.
- Two to three years experience in home health care or case management.. Bilingual (Spanish) a plus.
- Community Health Worker Certification must be completed within six (6) months of hire.
- Must have valid Ohio Driver’s License, reliable means of transportation for local travel and car insurance.
- Must have good dexterity with the ability to lift up to 30 pounds, push, pull, carry, climb, balance and kneel and the ability to sit, stand, walk, bend, and squat.
- Ability to work with diverse client population, community organizations as well as staff at all levels.
- Must be well organized, detail oriented and have the ability to maintain confidentiality.
- Must be able to work independently from general instruction.
- Physical examination by a licensed physician required.
- Final applicant is required to be fingerprinted to complete background check.
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
