Position Details:

The certified community health worker role will collaborate with healthcare providers in assisting patients with education and support to improve healthcare outcomes. They provide intensive case management to patients and assist with access to care, barriers in obtaining necessary resources; both community and insurance based. In addition, they will act as a guide in navigating transitions of care during a patient’s health care journey.
