Salary Range: $120,000 - $130,000 Quarterly Bonus Potential

We are actively recruiting a dynamic and experienced Director of Case Management for a growing acute care hospital in Johnston, Rhode Island. This is a key leadership role for someone ready to make an impact by guiding a team and advancing patient care outcomes in a newer hospital setting.
About the Role:
As the Director of Case Management, you’ll oversee a dedicated team of professionals focused on high-quality patient transitions and utilization review. This role requires strong leadership, collaboration with interdisciplinary departments, and a hands-on approach to operations.
Key Responsibilities:
- Provide oversight and mentorship to a team of 3 full-time case managers, 1 case manager assistant, and 2–3 per diem staff
- Manage staffing schedules and participate in the weekend leadership rotation (approximately once every two months)
- Lead a multidisciplinary case management team that includes RNs, a Physical Therapist, and Social Workers
- Monitor caseloads (average 12–13 patients per case manager) and maintain compliance with internal and regulatory standards
- Collaborate with leadership to enhance care coordination, discharge planning, and patient advocacy initiatives
- Drive the certification process, with increased compensation upon successful CCM or ACM certification within the first year
Qualifications:
- Background: Minimum 1 year of supervisory or leadership experience in a healthcare case management setting
- Credentials (choose one track):
- Registered Nurse: Must hold a BSN
- Social Worker: Must hold a Master's degree in Social Work or an active Social Work license
- Certification: CCM or ACM required within the first year of employment (salary increase upon completion)
