Position Details:
- This new position is supporting a busy women's health practice
- Provides appropriate assessment of patients and provides diabetes and nutritional education, counseling, and individualized plans in accordance with patient's nutritional goals
- Documents assessments, evaluations, and education and coordinates follow-up for individualized care plan
- Collaborates with Physicians, NPs and Midwives, and other Diabetes Educator to get the best results for our patients.
Job Requirements:
- Registered Dietitian or Registered Nurse
- Certified diabetes educator
Job Type: Part Time - .2 to .4; weekdays during normal clinic hours between 8:30AM-4:30PM
