CASE MANAGER
Part-Time, 29 hours a week
$18.37 - $22.34 per Hour

General Summary:
Under the supervision of the Director, collects information on clients and completes case coordination and support forms, assesses applicants’ needs for service, develops a service plan, and conducts periodic reassessments of each client. Makes referral to various service agencies and provides direct services to clients such as assist in completing applications for services, obtaining prescriptions, and related services.
Employment Qualifications
Education: Equivalent of high school graduation.
Experience: Some prior experience working with seniors in a casework or care capacity.
Other Requirements: Possession of a valid Michigan Driver’s License.

Working Conditions:
Works in office conditions and travels throughout the area to attend meetings and access and monitor clients in their homes.
Exposure to homes of various states of cleanliness and hygiene.
Hours are generally Monday – Friday from 8:00 a.m. – 4:00 p.m.
