Position Summary:
Assess the patient's psychosocial problems and needs at admission and ongoing thereafter. In collaboration with the patient, family, significant other and members of the treatment team, develops a plan for treatment and aftercare based on the identified , prioritized psychosocial issues. Specific social work services include discharge planning and coordination and patient education and counseling. Enables patients to utilize medical care in order to achieve their optimal level of health.
Requirements:
- Master's degree in Social Work required.
- NYS License in Social Work required.
- Minimum of one year of experience as a Social Worker required.
- Discharge planning experience
- Knowledge of Medicare Regulations
- Understanding of Managed Care Environment
- Excellent Interpersonal and Communication Skills
- Case Manager Certification a plus
