Job Overview:
We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) for a 13-week contract at an inpatient hospital setting. This full-time, day-shift role focuses on supporting patients in medical-surgical (med/surg) units, offering a rewarding opportunity to make a direct impact on patient care in a dynamic healthcare environment.
Key Responsibilities:
- Deliver psychosocial assessments and support services to patients and families in inpatient med/surg units.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to develop and coordinate individualized care plans.
- Provide crisis intervention, discharge planning, and referrals to community resources as needed.
- Ensure thorough documentation and adherence to hospital policies and regulatory standards.
Qualifications:
- Experience:
- Minimum of 1 year of LMSW experience, including experience in inpatient hospital medical/surgical settings.
- Education & License:
- Master’s Degree in Social Work.
- Valid LMSW license issued by the New York State Education Department (NYSED).
