Responsibilities
- Provide support with daily tasks such as cooking, cleaning, and laundry
- Assist with medication administration and medical appointments
- Support overall health and well-being
- Document activities and progress via web-based case management application (THERAP)
- Plan and participate in fun and leisure experiences for the individual
- Comply with all state and agency policies and procedures
Requirements
- Must be a resident of Arizona
- Valid driver's license, car insurance, and current registration
- Active home or renters insurance
- Own or rent your own home
- 21 years of age or older
- Reliable computer and internet access
- Ability to pass State/Federal and APS/CPS background check
