POSITION SUMMARY

Provide vision, leadership, and clinical care in the area of perinatal medicine at NAHC. Provide comprehensive primary health care to pediatric and adolescent patients.
The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).

DUTIES AND RESPONSIBILITIES
Perinatal Directorship
1. Provide clinical oversight for the perinatal program, including services provided by the Certified Prenatal Health Workers.
2. Oversee the operations of the perinatal program, including management of the Comprehensive Perinatal Services Program (CPSP), ensuring access and high-quality care is provided to perinatal and pediatric members and program performance goals are met.
3. Leads perinatal case conference meetings.
4. Ensure continuity of care of NAHC members with external specialists and hospitals.
5. Staying up to date in the area of perinatal care and disseminate information to NAHC clinical team.
6. In collaboration with the COO and Grants Management, seek funding opportunities that bolster perinatal and pediatric programs.
7. Represent and advocate on behalf of the perinatal program internally and externally.
8. Develop, implement and evaluate perinatal and CPSP specific services provided at NAHC, including workflows and policies and procedures.
9. Prepares audit tools, performs audits and quality improvement reviews, and analyzes Perinatal program statistics. Plans and implements program changes based on results.
10. Participates in management administration to enhance clinic efficiency and expand capacity at NAHC sites. Clinical Responsibilities
1. Under standardized clinical practice protocols, the FNP manages a panel of perinatal and pediatric, and adolescent patients.
2. Provide primary care to members, including histories, exams, lab tests, prescriptions and referrals within prescribed up to date guidelines and protocols.
3. Perform accurate and timely documentation of histories, subjective findings, physical exam, plans, prescriptions and follow up in Electronic Health Records System within 72 hours of the visit.
4. Provide members information concerning their evaluation, diagnosis, treatment and prognosis. Engage members in participation and decision involving their healthcare.
5. May provide direct supervision, guidance and support to NP/PA practitioners when appropriate.
6. Adhere to medical visit model of NAHC; managing huddles, proactive in teaching MA’s, seeing members on time, communicating with supervisors any barriers to this. Actively participate and be engaged in quality improvement for clinic efficiencies and improving member quality of care.
7. Timely coordination of care among internal and external ancillary providers and hospitals to provide high quality care between departments and facilities.
8. Function within an interdisciplinary team and participate in interdisciplinary activities such as panel management and case conferences.
9. Report births, deaths, and outbreaks of diseases to proper authorities.
10. Participate in the integrative model of providing medicine. This includes referring members and co-managing members with Nutritionists, LCSWs, etc.
11. Ensure completion of medication refills, provider approval queue, inbox items, medical record approval and member forms within five (5) working days.
12. Be an active educator of medical team members, including Nurse Practitioners, Physician Assistants, Medical Assistants, Nurses, and other integrative services team members.
13. Facilitate group interventions around site-driven topic areas in order to build capacity (i.e. obesity interventions, LGBTQ youth, etc.) as needed.
14. Perinatal: If assigned to perinatal program the following duties apply.
a. Provide comprehensive family planning, education, medication and infertility education.
b. Coordinate and collaborate care with OB and Pediatric specialists and Child Protective Services in appropriate counties.
15. Implement quality improvement initiatives and track outcomes to improve services.
16. Attend regularly scheduled site meetings with clinic and school staff to coordinate screenings and activities.
17. Work closely with BH site counselor, dentist and health educator to insure integrated, seamless services for identified youth.
18. Actively participate in creating solutions to increase access and build sustainability.
19. Work collaboratively with, and support the activities of the clinical data team, ensuring optimal accuracy and performance on clinical outcome measures.
20. Member Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe member practices.
21. Quality Improvement: Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.
22. Resolve staffing issues and facilitate conflict resolution for staff under direct supervision. Hire, discipline, evaluate and terminate staff in coordination with the Human Resources department.
23. Effectively lead and retain your staff by fostering a team environment and providing opportunities for staff to develop new skills
24. Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
25. Safety: Responsible for ensuring that all duties, responsibilities and operations are performed with the utmost regard for the safety and health of all personnel involved, including themselves.
26. Safety: Take appropriate corrective actions to address matters pertaining to employee health and safety that have been brought to their attention.
27. Works extremely well under pressure, meet multiple and often competing deadlines.
28. At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community.
29. Other duties as assigned by Supervisor.

Scope of Work
1. Be able to see between 16-20 members per day, and complete accurate documentation within 72 hours of visit into the electronic health record (EHR) system.
2. Be able to complete medical refills, member approval queue, inbox, medical records approval and member forms within five (5) working days.
3. To work as medical team assisting other providers in completing their responsibilities as necessary, e.g., seeing members, processing refills, clearing member approval queue, and provider inbox.
4. Effectively manage a member panel up to the goal set by the organization.
5. Display the highest level of professionalism, integrity and a strong work ethic, serving as a model for all clinic staff.
6. Maintain requirements of licensure, DEA certification and board certification if hired as board certified. Fulfill and maintain requirements for credentialing, privileging internally at NAHC and externally with payers and regulatory bodies.
7. Provide comprehensive, high quality medical services to members, regardless of age, sex, gender identification, income, national origin or language.
8. Consider individuals’ cultural, psychological, social and environmental factors in creating treatment/care plans.
9. Actively participate in meetings as assigned by direct supervisor.
10. Participate in trainings of other medical personnel as requested.
11. Participate in Saturday clinics, after-hour clinics and on-call duties, on a rotating basis, as requested by Supervisor.

MINIMUM QUALIFICATIONS


1. Current RN license and certificate of completion from an accredited NP program.
2. Five (5) years of experience as a practitioner in a community clinic setting preferred
3. Ability to function independently and as part of a team, including humor and grace under pressure.
4. Experience working with electronic health record systems, EPIC is preferable.
5. Experience working with population management systems, i2i is preferable.
6. Three (3) years of experience in a supervisorial position.
7. Program management experience, including budgeting, planning and development.
