What are the responsibilities and job description for the PACE Center Caregiver [FT/PT/On-Call] position at North East Medical Services?
Under the direction and supervision of the Center Manager, the PACE Center Caregiver provides personal care services to North East Medical Services PACE participants in the PACE Center, as well as other activities and tasks as assigned to assist different areas in the PACE Center, including rehab therapy, kitchen, transportation, and laundry.
ESSENTIAL JOB FUNCTIONS:
- Provides personal care, supervision, and escort assistance in accordance with each participant’s individual care plan.
Tasks may include peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation. - Reports observed changes in participants’ conditions to Center Manager and/or other center-based clinical staff.
- Proactively communicates participant health and safety concerns to Center Manager and/or other program staff.
- Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions.
- Interacts in friendly, courteous, and professional manner with participants and their families/caregivers.
- Participates in social and recreation activities with participants as appropriate.
- Provides assistance to participants based on primary area of assignment(s) within the PACE Center:
- Day Room: Supports participant engagement in the activity program under the direction of the Recreation
Therapist while meeting their personal care needs. - Rehab Therapy: Implements interventions, as appropriate to training, as directed by rehabilitation staff, including
use of mechanical assistance for participant transfers and mobility. - Kitchen: Assists with food service preparation and delivery and monitoring during mealtimes.
- Transportation: Accompanies drivers to pick up and drop off participants. Accompanies participants to outside
clinical appointments when necessary. - Laundry: Washes and dries Center and participant laundry. Coordinates getting participants’ laundry to/from the
participant’s home.
- Day Room: Supports participant engagement in the activity program under the direction of the Recreation
- Assists with covering assignments in other areas as needed for daily operations.
- Performs other job duties as required by manager/supervisor.
QUALIFICATIONS:
Required:
- Minimum 6 months of experience as a Personal Care Aide, Certified Nursing Assistant, or EMT.
- 1-year experience working with the frail or elderly population.
- Must complete and pass the North East Medical Services PACE training curriculum upon hire.
- Current documentation of CPR/First Aid — or willingness to obtain certification within three months of hire.
- Ability to communicate effectively.
Preferred:
- Knowledge of the geriatric population.
- Knowledge of safe and sanitary personal care techniques for elderly population with physical with physical limitations.
- Knowledge of proper body mechanics during provisions of assistance with Activities of Daily Living.
- Home Health Aide certification
- Certified Nursing Assistant (CNA) certification
LANGUAGE:
- Must be able to fluently speak, read and write English.
- Fluency in Chinese (Cantonese and/or Mandarin) is required.
- Fluency in other languages is an asset.
STATUS:
This is an FLSA Non-exempt position.
This is not an OSHA high-risk position.
