As the Project Evaluator, you will work within our Quality Department located in Mt. Holly, Burlington County NJ.
Schedule : Monday-Friday 8 : 30-5 : 00pm with 30 min unpaid lunch; Ability to work hybrid schedule
Responsibilities :
- Evaluating outcomes for the Certified Community Behavioral Health Clinic (CCBHC) including working with operational team to ensure data collection is valid and reliable, analyzing outcomes report data and running reports to inform team operation.
- Analyzing trends and working with CCBHC leadership to use data to drive performance improvement efforts, at both an individual and population health level.
- Ensuring that baseline interview data, reassessment interview data, and clinical discharge data into SPARS within required timeframe.
- Creating workflows with CCBHC program management to utilize and integrate data from external providers databases, such as the Health Information Exchange
- Running reports on key performance indicators to determine if required assessments have been completed,
- Working with CCBHC leadership to review clinical pathways and related clinical decision support tools, which will promote the best outcomes.
- Train CCBHC program management to run reports, interpret data, make conclusions, and shape curriculum, policy and procedures.
- Working with managers to evaluate the effectiveness of improvement initiatives
- Working with CCBHC leadership and MIS department to determine the best means of collecting and data and creating reports; and ensuring that the organization is able to report on key outcome measures.
- Working with administrative teams to guide data entry process
- Preparing the annual Programmatic Report for SAMHSA including accomplishments, actions taken to overcome barriers to accomplishing goals and progress for performance measures.
- Participating in the CCBHC Advisory Committee.
- Other job related duties as assigned.
