Position Overview:
As a Case Manager, you will play a critical role in providing support and guidance to individuals struggling with substance use disorders. Your primary responsibility will be to assess clients' needs, and collaborate with the clinical and medical team to carry out treatment plans based on clients needs, goals, and resources available. This role requires excellent communication skills, strong organizational abilities, and a deep understanding of the challenges faced by those battling with addiction. We are looking for someone who is compassionate, dedicated, and eager to grow.
Key Responsibilities:
- Provides support services to include engaging clients in case management and personalized aftercare plans.
- Helps prepare initial aftercare plan, event notifications and other documentation regarding clients in accordance with requirements, company policies and procedures.
- Monitors case load daily to ensure required documentation is completed timely; including, but not limited to, assessments, case reviews, discharge plans, legal documents, FMLA and Short term disability.
- Facilitates coordination of care to include anything clients require in their initial aftercare plan.
- Ability to be resourceful; seek appropriate resources for clients and maintain relationships with outside entities.
- Completes other tasks as assigned.

Salary: $38,000-$42,000 per year

Requirements
Qualifications:
- High School Diploma or Equivalent
- 1 year of Behavioral Health work experience (preferred)
- 18 years of age or older.
- Complete and pass pre-employment requirements, such as a background check and drug test.
Knowledge and Skills:
- Caring, Compassionate and Professional
- Knowledge of software programs for personal computer use, especially word processing, email, and spreadsheets.
- Strong problem-solving skills
- Excellent time management skills: Timely and consistent documentation, ability to carry out assigned projects to their completion and meet deadlines.
- Strong interpersonal skills, including the ability to communicate effectively with clients and team members both verbally and in writing, establishing, and maintaining effective working relationships, gaining cooperation, and resolving conflicts.
- Understanding of substance abuse treatment, rehabilitation, and range of treatment/interventions needed for chemically dependent individuals.
- Understanding of the 12-steps, Refuge Recovery, and other relevant recovery literature and principles of established support models.
Salary : $38,000 - $42,000
