What are the responsibilities and job description for the Case Manager-Bilingual preferred position at OLD COLONY YMCA?
Provide effective case management for children and families residing in Family Services programs including the David Jon Louison, the Family Life Center, and our Bolton Place Center. Case Management includes monitoring compliance with the Department of Housing and Community Development self-sufficiency plan, assessment of client strengths/needs; referrals and linkages to community based services; preparation to return to an independent living situation, and working with family in community after they have been rapidly re-housed.
