At One Vision, we believe in supporting individuals with disabilities to live meaningful, independent lives. We're currently seeking a compassionate, detail-oriented Outcome Coordinator to lead and integrate person-centered support plans and ensure quality care for individuals in our Home and Community-Based Services (HCBS) program.
Position Overview:
As an Outcome Coordinator, you will be responsible for coordinating and monitoring the Individual Support Plans (ISPs) for up to 25 individuals. You'll lead a multidisciplinary care team to ensure goals and services are aligned with the unique needs, preferences, and strengths of those we serve. Your work directly impacts the health, safety, rights, and overall well-being of individuals receiving our support.
Key Responsibilities:

Coordinate and implement ISPs, ensuring timely updates and accurate documentation.

Lead annual ISP meetings and collaborate with care teams, guardians, and case managers.

Monitor service delivery, document progress, and adjust plans to meet evolving needs.

Provide direct support and promote independence in daily living activities.

Train Direct Support Professionals (DSPs) and staff on plans, programs, and documentation.

Ensure services comply with federal, state, and OV guidelines, including Medicaid and accreditation standards.

Promote safety, human rights, and dignity of all individuals served.

Conduct quality assurance reviews of services documentation and intervene as needed.

Participate in staff orientation, ongoing training, and professional development.

What You'll Need:

BA/BS in Human Services or RN license, or equivalent years of experience supporting individuals with disabilities.

1 year of experience in disability services required.

Valid driver's license and reliable transportation.

Completion of Medication Administration training within 6 months of hire.

Strong leadership, organization, and communication skills.

Ability to physically support individuals (up to 50 lbs), respond to crises, and implement Mandt procedures.

Comfortable using Microsoft Office and documentation systems like Therap.
