One Vision is seeking to hire a Full Time - Smart Living Outcome Coordinator - Our Smart Living Outcome Coordinator will coordinate and integrate the Individual Support Plan in our Tech- Enabled Service homes in Clear Lake, Iowa.

Essential Job Functions:
- Lead care team in assessments for strengths, needs, preferences, supports, and goals.
- Coordinate annual ISP meetings, fostering open conversations and encouraging input.
- Monitor and complete quality assurance for HCBS intervention notes/services documentation.
- Prioritize safety and health by promptly addressing concerns, providing feedback and training on safety procedures, and reporting incidents.
- Ensure Behavior Support plans follow the least restrictive hierarchy, obtaining informed consent and human rights team approval before implementation.
- Maintains and ensures required electronic documentation is completed in a timely manner.
Required Skills/Abilities:
- Exhibits moderate computer skills including internet and Microsoft programs, such as Excel spreadsheets.
- Has legible writing for documentation required for role.
- Comprehend and implement legible written and verbal instructions to complete required paperwork and support people served in completion of their activities of daily living.
- Must be aware of details and accurately able to complete required paperwork and ensure compliance with federal and state rules.
- Able to hear sounds to support and respond to people served needs.
- A valid driver's license and proof of auto insurance is required to support persons served by providing transportation for medical appointments and activities in the community.
- Requires physical activity bending, kneeling, stooping. With the ability to lift up to 50lbs occasionally.
Job requirements:
- BA/BS degree in a human service-related field or RN license with 1 year of experience in supporting people with disabilities. Or 5 years of progressively responsible experience supporting individuals with disabilities/ equivalent education.
- Completion of OV Med Administration within 6 months of hire.
- Offer of employment is contingent upon pre-employment background check, and health screenings.
