Responsibilities:
- Deliver skilled nursing care in accordance with the plan of care established by the RN and physician
- Monitor and report changes in patient condition
- Administer medications, treatments, and wound care
- Collaborate with patients, families, and the interdisciplinary team
- Complete accurate, timely documentation on an EMR system
Requirements:
- Active LVN license in the state of California
- CPR certification (BLS)
- Valid driver’s license and reliable transportation
- At least 1 year of LVN experience (Home Health experience preferred)
- Excellent communication and time-management skills
