Registered Nurse (RN)

Position Overview:
We are seeking compassionate, experienced, and skilled Registered Nurses (RNs) to join our Home Health team. As a Home Health RN, you will be responsible for providing direct patient care and coordinating individualized care plans for clients recovering from illness, injury, surgery, or managing chronic conditions.

Responsibilities:

• Perform comprehensive Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge assessments.
• Develop and implement individualized patient care plans in coordination with physicians and other healthcare professionals.
• Educate patients and caregivers on disease processes, medications, and home care techniques.
• Provide wound care, medication administration, catheter care, and other skilled nursing services.
• Monitor patient condition and report changes to the physician and interdisciplinary team.
• Accurately document all services rendered in compliance with Medicare, state regulations, and agency standards.
• Participate in case conferences and quality improvement initiatives as needed.

Requirements:

• Active and unencumbered RN license in the state of [Insert State]
• 1 years of nursing experience required
• Knowledge of OASIS-E documentation strongly preferred
• Reliable transportation and valid driver's license
• CPR certification (BLS)
• Excellent clinical judgment, communication, and time management skills
• Ability to work independently in a home setting

Preferred Qualifications:

• Prior experience in a Medicare-certified Home Health setting
• Familiarity with EMR systems such as Kinnser, DSL, Devero, or Axxess
• Bilingual skills (Spanish, Tagalog, etc.) are a plus
