What are the responsibilities and job description for the Enrollment Associate position at PACE Organization of Rhode Island?

Job Summary:
Reporting to the Chief of Growth, the Enrollment Associate is responsible for enrolling potential participants into the PACE program. Identify opportunities to generate referrals. Ultimate goal is to drive potential participants and/or caregivers to the PACE program. Must be organized, courteous and able to work independently and with a team. Must be able to work on multiple tasks at once, with focus and sharp attention to detail.

  • Schedules intake appointments, and uploads completed Intake Assessment to a shared drive within 24 hours of visit
  • Verify program eligibility as stated in state and federal regulations.
  • Present key information to Intake Review Team on a bi-weekly basis. Ensure that focus is on information clear & succinct to Interdisciplinary Team in determining how to keep the participant safe in the community. This can be done via Zoom or in person.
  • Work with the Health Center Administrative Assistant to obtain a monthly schedule of providers to conduct Medical Assessments and schedule medical assessments accordingly.
  • In collaboration with Enrollment Administrative Assistant, ensure that participant chart is delivered to the respective PACE site 24 hours in advance of the scheduled Medical Assessment; so that Practitioner has ability to review records in a timely manner.
  • Collect any outstanding supporting documents required for Medicaid Application and submit to Enrollment Administrative Assistant.
  • Actively participate in all scheduled Enrollment Strategies for Intakes completed by respective Enrollment Associate-in person or via Zoom. Identifies any incomplete areas and communicates with Enrollment Administrative Assistant to ensure follow up for a complete Enrollment Strategy prior to Enrollment Conference.
  • In collaboration with the Social Worker, actively participate in the participant enrollment conference. Address any non-clinical questions regarding participant rights, responsibilities, and program regulations. Obtain a signed Enrollment Agreement
  • Represent PACE at various community meetings, trade shows, health fairs and other events as assigned by the Chief of Growth and/or the Chief of External Affairs.
  • In partnership with the Chief of Growth, build relationships with referral sources, including but not limited to hospitals, rehabilitation facilities, elder housing resident coordinators, social workers and visiting nurses who refer clients to PACE.

Qualifications:

  • High School Diploma or Equivalent, Required
  • Bachelor's Degree, Preferred
  • 1 to 3 years of administrative support experience, Preferred
  • 1 to 3 years of sales or case management experience, Preferred
  • 1 to 3 years of Health Care Industry Experience, Preferred
  • 1 year of experience working with a frail or elderly population, Preferred
  • Driver's License and Access to Reliable Transportation
