We walk alongside families as a trusted advocate and “in-town relative,” offering guidance, support, and personalized solutions that preserve dignity and independence.
About the Role:
Care Managers play a key role in helping older adults and their families navigate the emotional and practical complexities of aging. This position is ideal for someone with strong experience in dementia care, senior services, or care coordination who is deeply compassionate, highly organized, and skilled at building trusted relationships.
You will support clients by coordinating services, monitoring needs, advocating for their wellbeing, and communicating with families. While this is a non-clinical role, it requires professionalism, confidence, and strong judgment when helping families make care-related decisions.
This position is part time and may involve working with clients across Bluffton and Hilton Head Island.
Scope of Work and Key Responsibilities:
Client Support, Advocacy & Communication
- Serve as a warm, reliable point of contact for clients and families, offering an “in-town relative” level of support.
- Monitor for changes in needs and provide clear updates to families.
- Offer guidance and reassurance as clients and families navigate dementia-related changes.
Care Coordination
- Support the development and oversight of personalized care plans.
- Coordinate appointments and services as needed.
- Help ensure care remains consistent, organized, and aligned with the client’s goals.
Resource Navigation
- Identify helpful local programs, dementia resources, and reputable service providers.
- Make recommendations that support safety, engagement, and wellbeing.
Documentation & Professional Standards
- Maintain brief, accurate notes and time logs.
- Communicate concerns or updates promptly and professionally.
Ideal Qualifications
- Experience supporting individuals living with dementia or Alzheimer’s disease.
- Strong communication skills and comfort working with families.
- Background in senior services, care coordination, or a related non-clinical field.
- Organized, dependable, and able to work independently.
- Warm, compassionate approach with good judgment and healthy boundaries.
- Reliable transportation and ability to travel locally for client visits.
Compensation & Structure:
- Independent Contractor (1099) role - contractor is responsible for their own taxes, schedule, materials, and methods.
- Pay: Starting at $35 per hour
- Part-time, flexible hours set by the contractor.
- No employee benefits.
- Hours may begin as little as 0–3 hours per week with one client, with the opportunity to increase as new clients are added and as the contractor chooses to grow their caseload.
