What are the responsibilities and job description for the Specialized Care Coordinator position at Partnership for Children?

Partnership for Children of Essex (PCE) is currently seeking a Specialized Care Coordinator.

The Specialized Care Coordinator will help youth and families who experience child-on-child problematic sexual behaviors (PSB). The Specialized Care Coordinator will play an important role in helping youth and families get the support they need to heal from trauma and build protective factors. The Specialized Care Coordinator will advocate, support, and guide youth and families to obtain necessary treatment. This position will primarily work with families affected by PSB, both the child/youth and family harmed by PSB, as well as the child/youth and family who initiated PSB. This position also provides opportunities to gain skills in trauma informed and trauma responsive care while being part of a statewide initiative to improve access to medical and mental health services in New Jersey. 

Key responsibilities include:

  • Conduct intakes and screening of referrals received. 
  • Work with families, agencies, and clinicians to coordinate referrals for evidence-based trauma treatment. 
  • Educate community partners (schools, law enforcement, mental health providers, etc.) on the new therapeutic framework for supporting and healing in PSB cases. 
  • Manage and track referrals. 
  • Collect data to inform quality improvement and quality assurance efforts. 
  • Communicate with the administrator of the Child Treatment Fund regarding referrals, providers of services, and data. 
  • Participate in ongoing relevant training. 
  • Establish and maintain relationships with local resource providers. 
  • Attend local CAC (Child Advocacy Center) multidisciplinary team (MDT) meetings when appropriate. 

Qualifications: 

  • Embrace the mission, philosophy, and values of the Partnership for Children of Essex and demonstrates an ability to work in collaboration with various individuals and system partners. 
  • Minimum of Bachelor’s Degree with two years of experience required preferably with at risk children and families, in a related field (e.g., social work, counseling, psychology, psychiatric nursing, criminal justice, special education). Master’s Degree with one year of experience preferred. Professional license preferred. 
  • Experience in clinical assessment and child/adolescent development, with community-based experience preferred. 
  • Clinically and culturally competent, with the training and experience necessary to manage complex cases across child serving systems. 
  • Knowledge of the Essex County community preferred. 
  • Computer literate with excellent writing skills. 
  • Prior wraparound experience a plus. 
  • Knowledge about childhood trauma, complex trauma and its effects on children and families. 
  • Works well with families and child serving organizations and clinicians. 
  • Strong interpersonal and communication skills. 
  • Detail oriented. 
  • Must possess a valid driver’s license with a good driving record. 
  • Bilingual a plus. 

Travel:

  • 0 – 60% travel primarily within Essex County is expected. Travel may also extend to other parts of the state.
