What are the responsibilities and job description for the Supports Coordinator position at Partnership For Community Supports?

The Intellectual Disabilities Supports Coordinator will provide supports coordination for individuals who have intellectual disabilities and their families on the phone and in person, often performing home visits. The Supports Coordinator will ensure that the needs and wishes of the individuals are addressed and life opportunities are expanded.


Duties include: Developing person-centered plans, locating, coordinating and monitoring services and family driven supports for individuals with intellectual disabilities or autism.

  • To develop and enter Individual Support Plans into the Home and Community Services Information System within required time frames, assuring the active involvement of the individual receiving service and his/her team, and in keeping with all Federal, State, and City compliance requirements.
  • Make a difference in the lives of individuals who have disabilities.
  • Monitor the health and safety for individuals on assigned caseload.
  • Facilitate team meetings with individuals, families and providers of service.
  • Connect individuals with community resources and specialized services.

  • Upon completion of satisfactory initial employment period, salary will increase to $42,619.20 per year.

Requirements

  • A Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science; or two years’ experience as a County Social Service Aide 3 and two years of college level course work, which include at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service; or any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions; or a combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years’ professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.
  • Valid driver’s license and use of personal automobile required to complete home visits and program visits.
  • Computer skills and excellent communication and organizational skills needed.
  • Job requires a sensitive, caring individual with excellent verbal and written communication skills who wants to make a difference in the lives of individuals with intellectual disabilities and their families.
  • Some experience working with individuals who have disabilities preferred.
  • Bilingual ability a plus.
  • Access to a personal, secure, password protected home WiFi connection preferred.
  • Position also includes local travel.
