JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by managing an effective team to provide quality services.
QUALIFCATIONS:
- Bachelors Degree in Behavioral Health Discipline
- At least one year of experience in the provision of behavioral health treatment and/or rehabilitation
- Valid Driver's License required
- COVID-19 vaccination, or willingness to receive vaccine required
SPECIFIC DUTIES:
- Effectively supervise assigned residential sites within the BH Adult Residential Program
- Maintain compliance with Title 55 PA Code Chapter 5310: Community Residential Rehabilitation Services and Philadelphia DBH requirements
- Interview and evaluate individuals for admission to the program as assigned
- Supervise admission, assignment of residential counselors and discharge of residents approved by the program director
- Regularly observe and provide feedback to staff to ensure that their interactions with each resident are appropriate and consistent with the individual’s service plan
- Ensure individuals gain access to needed psychiatric, medical, social, residential, financial and other appropriate services
- Ensure individuals have access and availability to meaningful community activities appropriate to their ability and interests
- Ensure adequate individual and group skill teaching occurs to promote independent living skills
- Ensure that medications are monitored in compliance with PATH policies and procedures
- Coordinate services for residents, including performing case management and crisis management services, such as accompanying residents to various appointments as needed
- Ensure that all relevant information is communicated to the psychiatrist and other members of the treatment team
- Actively participate in concurrent reviews with DBH and members of each resident’s treatment team on a regular basis
- Monitor assigned residential sites to ensure health and safety of residents and inform PATH’s Facilities Department and Program Director of any facility problems in a timely manner
- Ensure cleanliness and orderliness of assigned sites
- Actively participate in weekly scheduling meeting with the Program Manager to ensure appropriate staff coverage at all residential sites within the program
- Other duties as assigned
