Qualifications:
· Achieved a bachelor’s degree in human services, Case Management, Social Work or equivalent, OR, 2 years of relevant hands on experience.
· Proven experience with Case Management in Assisted living or similar environment
· Experience working with adults, geriatric population, families, and external entities to facilitate services for residents. Experience with mental health populations is a plus.
· Knowledge of MLTCP and MLTCP conversions preferred.
· Maximums and Medicaid experience a plus.
· Strong attention to detail
· Pleasant demeanor, compassionate, and strong advocate for resident’s needs.
· Must be able to communicate in English
· Must be reliable and able to work in fast paced environment.
Responsibilities:
· Advocating for the needs and concerns of the residents
· Assist residents with making and scheduling appointments
· Establishing relationships and resources with medical providers.
· Coordinate transportation, assist with personal paperwork/forms
· Spearheads solution-based initiatives and effectively produce favorable outcomes.
· Provide timely communication and information to family members
· Collaborate closely with residents, their families, and staff to create safe, nurturing, and high-quality living experience.
· Through documentation of ongoing needs and concerns of the residents.
· Monitor, observe, and report daily events of residents.
Working Conditions:
· Must be able to lift, carry, push, and pull up to 25 pounds frequently, including assisting residents with mobility and transferring.
· Requires the ability to bend, kneel, crouch, and reach as needed to assist residents with daily living activities.
· Must be able to stand and walk for extended periods, often while assisting or monitoring residents.
· Ability to safely maneuver residents, medical equipment, and supply carts throughout the facility.
· Emergency Response: Must be prepared to handle emergency situations, such as contacting emergency services or notifying appropriate staff in case of an emergency or security concern
· Must be able to verbally and physically, deter residents that are an elopement risk from leaving the facility
· Must be able to follow residents on grass or uneven surfaces to attempt to prevent resident elopements and ensure resident safety.
