Case Manager
DESCRIPTION: The Case Manager is responsible for providing case management services, including assessing clients’ needs, conducting psychosocial evaluations, developing individual treatment plans and maintaining daily progress notes.
REPORTS TO: Case Manager Supervisor
GENERAL RESPONSIBILITIES
Provide case management services.
- Provide education and support to clients and their family members.
- Conduct psychosocial evaluations.
- Develop individual treatment plans.
- Coordinate client care with the treatment team.
- Make appropriate referrals to outside community agencies as needed.
- Will require some travel to client’s homes and facilities in the community, and other surrounding communities.
- Monitor all aspects of client care and prepare and maintain client records in accordance with county, state and federal statutes.
- Knowledge of addictions, recovery, mental health and other co-occurring disorders.
- Excellent interpersonal skills and the ability to maintain a positive collaborative relationship with all staff.
- Experience in crisis intervention and conflict resolution skills.
- Ability to work in a fast-paced, dynamic environment.
- Experience working with an ethically, culturally and racially diverse client population; ability to work harmoniously with a wide diversity of individuals.
- Represents the center in a professional manner within the community, including being a liaison with other behavioral health facilities, medical health facilities, area schools and community agencies.

- Please note that this is an in-person position not remote.
Qualifications
- A Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education or a related human service field, and one (1) year full-time or equivalent experience working with children and adults with severe mental illness and/or substance abuse/addiction.
REQUIREMENTS
- Negative TB Test (must be renewed annually)
- CPR & First Aid Certification (Must be renewed every two years)
- Valid driver’s license and proof of insurance (Must be maintained throughout employment)
- Undergo a criminal background check
- Employment references check
- Must provide a copy of College transcripts/College diploma
HOURS OF THE POSITION
The Case Manager hours will be Monday through Friday 8:00am to 5:00pm with a one (1) hour unpaid lunch. There may be schedule changes to meet the needs of the program which may require some evening work. Case Managers will be expected to have at minimum 32 billable hours a week .
