Responsibilities
- Conduct initial assessments of clients' needs and develop personalized care plans
- Coordinate and schedule home care services based on clients' requirements
- Collaborate with clients, their families, and healthcare professionals to ensure the delivery of high-quality care
- Monitor the implementation of care plans and provide ongoing support and guidance to caregivers
- Assess caregiver performance and provide feedback and training as needed
- Maintain accurate and up-to-date documentation of all client interactions and caregiver assignments
- Stay informed about industry trends and best practices in home care services
- Bachelor's degree in a related field or equivalent work experience
- Minimum of 2 years of experience in home care coordination or a similar role
- Proven ability to assess clients' needs and develop customized care plans
- Excellent organizational and time management skills
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficient in using computer applications and software for documentation and scheduling
