What are the responsibilities and job description for the Patient Navigator position at PRAIRIE VIEW?
The full time Patient Navigator is responsible for collecting, managing, and reporting required data related to CCBHC required outcomes and measures which include National Outcome Measures (NOM's) and required reporting measures for CCBHC. This requires both interpersonal and data management skills. The Patient Navigator assures that patients are connected with the right program and services and seeks to close referral gaps that patients may encounter while receiving care in the CCBHC clinic. The patient navigator must be able to work collaboratively with clients, families, internal programs, external providers and all aspects of the CCBHC network. The Patient Navigator must be an outcome focused individual that seeks to help clients have an effective and efficient experience while receiving care at the CCBHC.
Minimum Experience: Experience working directly with behavioral health populations preferred.
Qualifications:
Good listening/verbal/written skills required.
Must be able to observe patient demeanor and non-verbal communication for assessment purposes.
Must be able and willing to work flexible hours and adjust work schedule to meet patient and organization needs.
Must hold and maintain State of Kansas Case Manager Certification
Work independently or as a productive member of a team to develop a therapeutic rapport with diverse individuals.
Must have a valid Kansas driver's license.
Must be proficient at basic keyboarding and math skills.
Must be able to pass required background checks.
